Patient Safety Forum (Houston Site)
This monthly one-hour casual meeting at our main office in Houston invites patients,
families, students and anyone who is curious or interested in discussing patient
safety issues and concern. The Forum has an open, interactive format with a facilitator
to present a brief overview of a topic in a slide presentation and to guide the conversation
and discussion. Monthly topics range from case study to current news story and patient’s
recent experience, covering the patient safety spectrum including:
- Cost and impact of medical errors
- Taxonomy and classification of near misses and errors
- Patient safety reporting systems for patients and families
- Dangers of unused and expired medicines
- Prevention of hospital-acquired infections
- Effective communication with doctors and nurses
- Patient Safety Checklists

- Patient-centered medical community model
- Complementary and alternative medicine
- Lessons from commercial aviation in safety
We provide participants additional materials to reinforce key points and take-home
notes, as well as light refreshments to enhance the Forum. Many attend our Forum
